4 Patients Injured After Drug Mix-Up at Ascension Hospital in Nashville

A pharmacy error at an Ascension hospital in Nashville left four joint replacement patients injected with potassium phosphate instead of spinal anesthetic. The mix-up paralyzed one patient and harmed the other three. The Tennessee Bureau of Investigation is now probing the case.
